How does abortion stigma work?

Abortion-related stigma is one of the primary factors that places safe, legal and accessible abortion care and services out of reach for individuals worldwide,[1] particularly in the Global South and particularly for young, poor, and unmarried women. Moreover, while socio-cultural factors entail that abortion stigma may take different forms in different places, abortion stigma is a global issue, demonstrated by the rise and continuance of legal and policy restrictions in places as diverse as the United States, Spain, Lithuania, Nigeria, and El Salvador, among others.

One of the ways in which abortion-related stigma plays an ongoing role in the social, medical, and legal marginalization of abortion care worldwide, is through the propagation of myths and misinformation surrounding abortion.

MYTH: “Abortion is dangerous.”

The reality is that surgical abortions done by trained providers in hygienic settings, and early medical abortions, are extremely safe procedures. In the United States, for example, where abortion is legal on broad grounds, less than 1% of all patients experience a major complication, and the risk of death associated with abortion is 10 times lower than that associated with childbirth.

MYTH: “Most service providers object to abortion.”

It is a common myth that most service providers globally object to performing abortions, regardless of the abortion legislation in the country. Clearly, there is a high number of service providers who do not perform abortions in countries where it is illegal. Nevertheless, several studies show that a vast majority of obstetrician-gynecologists (ob-gyns) and other healthcare providers globally support abortion at least in some circumstances. National, regional and international ob-gyn societies are often active in advocating for policy change for safe and legal abortion, disseminating information, creating awareness and encouraging ob-gyns to support individuals’ access to safe, high-quality services.

Numbers of evidence-based studies have shown that criminalizing and/or restricting access to abortion does not reduce the number of abortions—rather, it forces individuals to seek out clandestine and unsafe abortions worldwide, jeopardizing their health and their lives.
